Online Registration (New Student)

2025-2026 Online New Student Registration Requirements

Students who are registering in the District for the first time must upload the following:

An Original/Certified Birth Certificate
Physical Examinations and Immunizations (required for all pre- k, kindergarten, 6^th grade students and ALL students new to the district)
Dental examinations are required for kindergarten, 2^nd and 6^th grade students.
New students are required to have an eye examination by a licensed physician or optometrist.
All pre-k and 6^th grade students must show proof of having the Hepatitis B vaccination series.
Pre-K students must have at least 1 dose of Varicella and MMR. K-8^th need two doses of MMR and show proof of two varicella vaccinations (chicken pox vaccine) prior to registration.
One dose of Meningococcal Vaccine (MCV) and one dose of Tdap vaccine is required for 6^th, 7^th, & 8^thgrades.
ISBE Transfer Form, (required for all students transferring from an Illinois public school)
Students transferring from a private school must have a withdrawal and grade placement documentation (private schools will not release information if fees are owed).
Students who are receiving Special Education Services must upload a copy of the current (dated no later than one year) Individualized Education Program (I.E.P.).
Custody Documentation (if applicable).

To Prove Residency, Parents Must Upload
