Online Registration (New Student)

2026-2027 Online New Student Registration Requirements

Students who are registering in the District for the first time must upload the following:

  • An Original/Certified Birth Certificate
  • Physical Examinations and Immunizations (required for all Pre- K, Kindergarten, 6th grade students and ALL students new to the district)
  • Dental examinations are required for Kindergarten, 2nd and 6th grade students.
  • New students are required to have an eye examination by a licensed physician or optometrist.
  • All Pre-K and 6th grade students must show proof of having the Hepatitis B vaccination series.
  • Pre-K students must have at least 1 dose of Varicella and MMR.  K-8th need two doses of MMR and show proof of two varicella vaccinations (chicken pox vaccine) prior to registration.
  • One dose of Meningococcal Vaccine (MCV) and one dose of Tdap vaccine is required for 6th, 7th, & 8th grades.
  • ISBE Transfer Form (espanol), (required for all students transferring from an Illinois public school) 
  • Students transferring from a private school must have a withdrawal and grade placement documentation (private schools will not release information if fees are owed). You must certify transferring students are not currently required to serve a suspension or expulsion or avoid retention imposed by an in/out-of-state public/private school, from which the student is transferring.
  • Students who are receiving Special Education Services must upload a copy of the current (dated no later than one year) Individualized Education Program (I.E.P.).
  • Custody Documentation (if applicable).

**Students who reside outside the district boundaries and wish to attend one of our schools are required to pay tuition in the amount of $17,617** Please contact our district at 708-210-2888 for more details. 

To Prove Residency, Parents Must Upload

Two (2) Utility Bills (showing current in district address and no more than 30 days old)

  • Gas
  • Electric
  • Water/Sewer
  • Cable
  • Scavenger/Garbage

                 AND

 (Current and showing in district address)

  • Illinois Driver’s License / Illinois State I.D.
  • Mortgage/Lease Agreement
